Community Alternatives Program

In-Home Care


Community Alternatives Program (CAP) is an in-home care program offered by Unity Home Care that allows patients to be cared for at home and receive many more hours of care than they would receive under a PCS plan. 

To receive CAP services, you’ll need to schedule an in-home evaluation with your local CAP office. The CAP office will send over a certified specialist to provide a full evaluation and report back to the office about whether the patient needs extended care.

Be aware that availability for this program can be limited and you may have to be put on a waiting list for an opening.
